Adverse Action Notice Template

Required by the Fair Credit Reporting Act when denying a rental application based on a consumer report.

Legal Requirement: Under the Fair Credit Reporting Act (15 U.S.C. § 1681m), if you take adverse action against an applicant based wholly or partly on information in a consumer report, you must provide the applicant with an Adverse Action Notice. Failure to do so may subject you to civil liability.
